Step 2 - Get a Memorization Affidavit

You may call us at (615) 883-1687 and we will mail a memorization form to you. Be sure to speak distinctly. Give you name, address, city, state, zip code and telephone number. Then repeat the information so that we will be sure to get it correctly.
This Affidavit is filled out with the child's name, address and signature to state they have memorized and recited the Ten Commandments to an authorized witness. |
Step 3 - Authorized Witness
The authorized witness can be a pastor, rabbi, church school teacher, a youth leader, an elected government official, a qualified candidate for public office or a person authorized by The Ten Commandment Project's staff. The authorized witness also fills out their section of the Affidavit to validate the genuineness of the efforts. |
Step 4 - Choose a plan
Choose a scholarship rewards plan. Be sure to fill out the form completely. |
Step 5 - Mail the Affidavit
Mail the fully completed Memorization Affidavit to The Ten Commandments Project:
