| A “Memorization Affidavit” can be obtained by contacting The TEN COMMANDMENTS PROJECT office. Request may be made by phone (615) 883-1687, by U.S. Mail (2200 Lebanon Road, Nashville, TN 37214) or it can be downloaded from our web site, www.tencommandmentsproject.org. |
| Send the completed affidavit and a $10 check for your particular child. Make your check payable to TEN COMMANDMENTS PROJECT or, if you prefer, make it in the name of your particular child and it will be returned along with the other benefits. |
